  • The Guest Registration Form provides the Jazz Cabaret Committee with the information needed for:

         - Entrée choices

         - Seating assignments

         - Allergy information

  • All tickets must be purchased by Friday, January 3, 2020.
  • Tickets will not be sold at the door.  
  • Advance meal reservations are required, including meals for musicians.
